Cards must be shown at registration

An electronic version of the registration form is available either in Excel format or in PDF format, allowing you to fill it out in advance. A paper version will be available at the registration sessions.

If you have Microsoft Excel

If you have the Microsoft Excel software, you can fill out the form electronically, print it and bring it to one of the registration sessions.

Please follow the following steps:

1. Read the registration conditions (see above). If you accept the conditions, please continue;

2. Click here to download the form in Excel format; (unless you already received it by email)

3. Fill out the form with Excel (yellow fields only). Trick: press Enter to get to the next field;

4. Print one copy;

5. Sign it;

6. Bring the form to one of the registration sessions along with the payment and the cards (Acces, Medicare) and other documents required if U13 and up.

Note: Completed forms cannot be droped at the Elgar center.

PDF  format

If you do not have access to the Microsoft Excel software, you can dowload the form in PDF format, print it, fill it out and bring it to one of the registration sessions.

Please follow the following steps:

1. Read the registration conditions (see above). If you accept the conditions, please continue;

3. Print 1 copy;

4. Fill out the form (yellow fields only);

5. Sign it;

6. Bring the form to one of the registration sessions along with the payment and the cards (Acces, Medicare) and other documents required if U13 and up..

No Printer

If you cannot download the forms or if you do not have access to a printer, forms will be available at the registration sessions. Please read carfully the registration conditions beforehand.

Late registration

There are a few spaces left in the following divisions:

- Boys U9-10 (born in 1999 or 2000)

- Boys U13-14 (born in 1995 or 1996)

- Girls 7-8 (born in 2001 or 2002)

- Girls 11-12 (born in 1997 or 1998)

  All other divisions are full

Late registrations will be accepted  depending on availability .

You are required to follow each step of the procedure:

1. Request via email

    Send an email to inscription@soccerverdun.com and indicate the player's name, sex, date of birth and a telephone number

2. Availability

    We will verify if there is a space available in the player's division

3. Registration forrm

    If there is a space available, you will receive a registration form that you will be required to fill out electronically (late registration forms are not available at the community center).

4. Codes of conduct

    Read the codes of conduct that parents and players must conform to

5. Fill out the form

   Fill out the form using the Microsoft Excel program. Fill out all yellow fields. (If you do not have access to Excel, please mention it in your email)

6. Send the form via email

   Send the completed form ro inscription@soccerverdun.com

7. Approval

    The electronic form will be validated and we will confirm the amount to pay. If the form is incomplete, you will receive a request for the missing information.

8. Print and deliver the documents and payment

    Steps to follow:

 NOTE: only registration forms pre-approved via email will be accepted

• Print the completed form

• Sign the form

• Players U13 to U18 (born before 1997) :
  - recent photo required (small format)
  - photocopy of the medicare card on a full sheet

• Put all the documents in an enveloppe

• Include the exact payment in cash only (no checks accepted for late registrations)

• Important: for the 20$ rebate, only a valid GREEN Access card of the player is accepted and it must be presented.

• Drop the registration package to the following address: 210 Corot (townhouse North of blvd Ile-des-Soeurs)

• Bring the player's medicare card and his/her Access card (if she has one)

9. Confirmation

    You will be notified of the final approval of the registration. If player formation has already been made, an evaluation of the player will be required before assigning him to a team.
